You need to file a “Petition for Account Deletion” with the “Account Review Board.” Appendix 14 of the Petition includes a 20-page questionnaire, which must be signed under oath. You must also provide blood samples, photos (for each of the past 10 years), and a psych evaluation.

To delete membership, simply click on the 'delete account' box on your home page. :)
